
Disclaimer of Warrant/Limitation of Liability:
The consumer report obtained by Cornerstone Applicant Screening is derived from databases and records that have been created and maintained by various government agencies, private companies, and other contributors that are not under the control of Cornerstone Applicant Screening. Responsibility for the accuracy of the information contained in the consumer report and these databases and records rests solely in the contributor. The Subscriber waives any and all claim or claims against Cornerstone Applicant Screening arising out of or related to the accuracy of the consumer report, databases and records.
DESCRIPTION OF SERVICES
KBI Criminal Database:
Cornerstone Applicant Screening shall provide the Subscriber with the following types of adult conviction criminal history found by conducting a search of the KBI database:
• Court convictions for violations of Kansas law that are felonies or class A or class B
misdemeanors. Class C misdemeanor assaults are also part of the database.
• Court convictions for violations of municipal ordinances or county resolutions that are equivalent to class A or class B misdemeanors or class C misdemeanor assaults under state statute.
• Confinements in Kansas Department of Corrections facilities.
• Records of arrests within the past 12 months are also released when the records of disposition have not yet been received.
• Active diversions that have not yet been successfully completed.
• Printout of any record found under the Employee/Applicant/Participant’s name will be provided to the Subscriber for comparison to the Employee/Applicant/Participant.
KS County District Court:
Cornerstone Applicant Screening shall provide the Subscriber with County District Court records from either Civil or Criminal/Traffic or both, by conducting a search of the Kansas County District Court database. Please note: Not all Kansas County District Courts are represented. Cornerstone Applicant Screening will notify the Subscriber should a particular county's records be unavailable and no fee shall be charged to the Subscriber. Civil court records include, Domestic/Family Law, Civil Limited Action, Small Claims, Probate, Statutory Lien, State Tax, Personal Property Tax, Marriage License, and Miscellaneous Civil. Fees are assessed for each court search, each name and each case number.
Nationwide Sex Offender Registry:
Cornerstone Applicant Screening will conduct a nationwide search of the Sex Offenders Registry for each Employee/Applicant/Participant found to have either a current or previous addresses outside the State of Kansas. Since the inquiry is by name only, a printout of any record found under the Employee/Applicant/Participant’s name will be provided to the Subscriber for comparison to the Employee/Applicant/Participant.
Kansas Registered Offenders Registry:
Cornerstone Applicant Screening will conduct a Kansas statewide search of the Registered Offenders Registry. Since the inquiry is by name only, a printout of any record found under the Employee/Applicant/Participant’s name will be provided to the Subscriber for comparison to the Employee/Applicant/Participant.
Kansas Department of Corrections Inmate Search:
Cornerstone Applicant Screening will conduct a DOC search for person/s who are, or have been, incarcerated in one or more KDOC facilities, who are parole absconders, community corrections absconders and escapees. A printout of any record found under the Employee/Applicant/Participant’s name will be provided to the Subscriber for comparison to the Employee/Applicant/Participant.
Kansas Department of Motor Vehicles Search:
Cornerstone Applicant Screening will conduct a Kansas DMV records search and provide a history of violations, suspensions, and any other details available from the DMV about an individual's driving history.
Federal Criminal Records:
The Federal Criminal Records search directly accesses the Federal crime repository to identify criminal convictions on Federal charges including, but not limited to, securities and tax law violations, immigration violations, embezzlement, terrorism, weapons violations, interstate drug crimes and civil rights violations. Federal crimes tend to be very serious in nature.
Federal Criminal Record requests have a potential outcome of “Clear” or “Hit”.
• “Clear”: A search of the Federal District Court’s (selected by the Subscriber) index did not find a subject (within the scope of the search) whose identifiers matched the candidate information submitted by the Subscriber. Clear means the candidate did not have a record on file at the Federal District Court searched by Cornerstone Applicant Screening Services. It does not mean that the candidate does not have a record anywhere else.
• “Hit”: A search of the Federal District Court’s (selected by the Subscriber) index did find a subject whose identifiers matched the candidate information submitted by the client. NOTE: Federal records historically do not contain identifiers (DOB, SSN) – accordingly, most matches are Name Match Only. The record information will be provided to the Subscriber. A hit returns records from the Federal District Court searched. It does not preclude the possibility that the subject has additional records in courts not searched by Cornerstone Applicant Screening.
Employment Verification:
Cornerstone Applicant Screening will contact, by telephone or in writing, any number of Subscriber-requested employers. Information that will be obtained includes, starting date, termination date (if applicable), position, average hours worked per week, gross wage or salary information, and eligibility for rehire. Date information was obtained and person who provided the employment information will be provided. In any written request, a verification letter will be mailed to the employer and the Subscriber will be provided the employer’s written response. In any telephone request, the Subscriber will be provided a summary of the information requested via the Consumer Report. Wage information may not be attainable via telephone verification only.
Personal Reference:
Cornerstone Applicant Screening will contact, by telephone or in writing, any number of Subscriber-requested personal references. Information that will be obtained includes relationship to Employee/Applicant/Participant, length of relationship and any other comments provided by the person(s) named as a personal reference. The Subscriber shall provide Cornerstone Applicant Screening a list of questions to be asked of the personal reference.
Landlord Reference:
Cornerstone Applicant Screening will contact, by telephone or in writing, any number of Subscriber-requested landlords. Information that will be obtained includes dates of tenancy, if terms of lease agreement were fulfilled, any known violations of the lease agreement (including non-payment of rent, Community Policies, Drug-Free Addendum, etc.), name of all family members who resided with the Employee/Applicant/Participant, if proper notice to vacate was provided, number of times rent payment received late, amount of monthly rent, if any rent was federally subsidized, if Employee/Applicant/Participate vacated owing no monies for damages and/or unpaid rent, whether landlord would rent to the Employee/Applicant/Participant in the future and any other information volunteered by the landlord regarding the Employee/Applicant/Participant.
HA Query: (PHAs only--certain conditions apply)
Cornerstone Applicant Screening will contact HUD’s PIC database to inquire if Applicant/Participant has been a program participant elsewhere in the United States.
PHA Verification of “Good Standing” status:
Cornerstone Applicant Screening will contact, via telephone or in writing, any number of Public Housing Authorities for which an HA Query produced information that the Applicant/Participant has been a program participant. Information that will be obtained includes the name of the PHA, location (city and state) of the PHA, program name (HCV Section 8, Public Housing, FSS, Welfare-to-Work, etc.), dates of participation, name of all family members residing with the Applicant/Participant, any violations of HUD’s Family Obligations, whether Applicant/Participant exited the program in good standing and if the Applicant/Participant is eligible for re-admittance into the program.
In the event of the Applicant/Participant’s participation in the Public Housing program, upon request of the Subscriber, a Landlord Reference may be substituted or requested in addition to a PHA's verification. Applicable charges apply.
Written Verification of Assets:
Cornerstone Applicant Screening will contact, in writing only, any number of financial institutions which the Applicant/Participant has designated as having assets located at. Assets include, checking and/or savings account, certificate of deposit, IRA, annuity, etc. A verification form will be sent to the financial institution requesting information including, the type of asset(s), account number(s), 6-month average balance for checking account(s), current balance of all other assets, amount of any disbursement(s) to the Applicant/Participant during the last twelve (12) months, amount of interest earned in the last twelve (12) months, and annual interest percentage the asset earns, if any.
Subscriber will provide Cornerstone Applicant Screening the Applicant/Participant’s signed Authorization for the Release of Information and Privacy Act (form HUD-9886) in order to conduct this service.
Written Verification of Other Income:
Cornerstone Applicant Screening will contact, in writing only, any number of other sources of income listed on the Applicant/Participant’s application for housing assistance and/or recertification application. Other sources of income that will be verified may include SRS benefits, child support, family support, self-employment income, pension, unemployment benefits and any other countable income as defined by HUD for each family member.
Subscriber will provide Cornerstone Applicant Screening the Applicant/Participant’s signed Authorization for the Release of Information and Privacy Act (form HUD-9886) in order to conduct this service.
KSBHA License Verification:
Cornerstone Applicant Screening will verify licenses issued by the Kansas State Board of Healing Arts. Professions regulated by the KSBHA: Medical Doctors, Osteopathic Doctors, Chiropractic Doctors, Podiatric Doctors, Physicians' Assistants, Physical Therapists, Physical Therapist Assistants, Occupational Therapists, Occupational Therapy Assistants, Respiratory Therapists, Athletic Trainers, Naturopathic Doctors, Contact Lens Distributors, Radiologic Technologists.
KSBN License Verification:
Cornerstone Applicant Screening will verifiy licenses issued by the Kansas State Board of Nursing. The KSBN issues licenses for the following: Registered Nurse (RN), Licensed Practical Nurse (LPN), Licensed Mental Health Technician (LMHT)
KDHE Health Occupations Certification and/or License Verification:
Cornerstone Applicant Screening will verify licenses issued by Kansas Department of Health & Environment for individuals holding the following licenses, whether active or expired: Adult Care Home Administrators, Audiologists, Dietitians and Speech-Language Pathologists. Certification status is also available for nurse aides, home health aides, and medication aides.
KS BSRB License Verification:
Cornerstone Applicant Screening will verify licenses issued and regulated by the Kansas Behavioral Sciences Regulatory Board. Professions regulated by the BSRB: Psychologists (Ph.D.), Social Workers, Professional Counselors, Masters Level Psychologists, Marriage and Family Therapists, Alcohol and Drug Abuse Counselors.
NOTE: Any other type of verification request required by the Subscriber, and the price of such verification request, must be negotiated and agreed upon by both Cornerstone Applicant Screening and the Subscriber. The additional service(s) will be incorporated into the Client Agreement entered into prior to such service(s) being rendered.
In all services rendered by Cornerstone Applicant Screening, reasonable efforts will be made to obtain the information. In the event verification has not been received by any source after three (3) reasonable attempts, Cornerstone Applicant Screening shall disclose to the Subscriber the failed verification and provide written documentation as to its efforts to obtain the information; however the charge for such services will not be waived.
